Canine Freeze-Dried Plasma: A Game-Changer for Veterinary Trauma Care

A new solution for veterinary trauma care is emerging, offering a practical alternative to traditional plasma treatments. Canine freeze-dried plasma (cFDP), developed by Mantel Technologies, is designed to address the logistical challenges of administering fresh frozen plasma (FFP) in remote or austere environments. Unlike FFP, which requires frozen storage and thawing before use, cFDP can be stored at room temperature and reconstituted with sterile water when needed. This innovation could revolutionize trauma care for dogs, particularly working dogs in the field. ## How cFDP Compares to FFP Canine freeze-dried plasma (cFDP) is essentially FFP that has been freeze-dried and can be reconstituted as needed. Research has shown that cFDP is bioequivalent to FFP, meaning it provides the same efficacy in controlling bleeding and supporting coagulation. When reconstituted with sterile water, a 250 mL unit of plasma is produced, ready for administration. This eliminates the need for a cold chain, making cFDP a more practical option for veterinary professionals working in challenging environments. ## Safety and Hypersensitivity Reactions While cFDP offers significant advantages, safety studies have shown that approximately half of the dogs in the trials experienced mild hypersensitivity reactions. These reactions included hives, facial swelling, nausea, and occasional vomiting. However, no delayed transfusion reactions were observed, and the reactions were managed without severe complications. Dr. Kelly Mann, a veterinary radiologist and vice president of veterinary solutions at Mantel Technologies, emphasized that these reactions are not unique to cFDP. Similar reactions can occur with any blood product administration. The Association of Veterinary Hematology and Transfusion Medicine (AVHTM) guidelines provide recommendations for managing these reactions, including the use of antihistamines, glucocorticoids, and epinephrine when necessary. Mann also noted that the cFDP product itself does not appear to be the cause of the reactions. In vitro studies, including flow cytometry and proteomics, found no differences between cFDP and the FFP it is derived from. This suggests that the reactions are likely due to the individual dog's immune response rather than the product itself. ## Potential Applications and Future Directions The primary focus of cFDP's development was to support working dogs in austere environments, such as military working dogs. However, its potential applications extend to civilian veterinary care, including prehospital and emergency settings. Mann highlighted that cFDP could be particularly useful in situations where immediate access to FFP is not feasible. Looking ahead, Mann sees cFDP as a stepping stone toward developing shelf-stable blood products for dogs. This could include exploring the therapeutic effects of cFDP on conditions such as traumatic brain injury (TBI) and polytrauma. While current research is ongoing, the potential for cFDP to improve outcomes in these cases is promising. As with any new medical product, further research and clinical experience will be needed to fully understand the benefits and limitations of cFDP. However, its introduction represents a significant advancement in veterinary trauma care, offering a practical and effective solution for managing bleeding in injured dogs.
