A new study has identified a rare bone condition in a small number of dogs treated with the osteoarthritis pain drug Librela. The research, published in the Journal of the American Veterinary Medical Association (JAVMA), describes abnormal periarticular bone remodeling (APBR) in 186 dogs out of an estimated 34 million doses dispensed.

Richard Goldstein, DVM, global chief medical officer at Zoetis, discussed the findings with dvm360. He stated that the company opened its pharmacovigilance database to an independent panel of experts to characterize the condition. Goldstein explained the company's goal was to ensure all this data gets out as quickly as possible to the community.

What the Study Found

The study, led by Steven C. Budsberg, DVM, and colleagues, is the first to formally describe APBR. This syndrome involves new bone formation around the joints, not on the joint surface itself. The research team reviewed cases from Zoetis's safety database where radiographs were available.

Goldstein emphasized the context of the numbers. The 186 identified cases represent a tiny fraction of the millions of dogs treated. He acknowledged there may be more unreported cases but suggested the total is likely still very small. The exact role of Librela in causing APBR remains unknown. Goldstein noted that the exact influence of Librela on these dogs is not yet known.

Expert Review and Transparency

Zoetis assembled a diverse group of specialists to analyze the data independently. The team included veterinary surgeons, radiologists, pathologists, and experts in human orthopedic disease. Goldstein said the company took a hands-off approach, allowing the experts to form their own conclusions for publication.

The decision to include human medicine experts was strategic. Their extensive experience with pharmacovigilance databases and unusual findings was deemed valuable for understanding the canine cases. The goal was complete transparency. Goldstein stressed that every dog is important, even when events are uncommon.

Guidance for Veterinary Professionals

Goldstein stated that information about APBR should be part of the standard conversation veterinarians have with pet owners about adverse events. He framed the discussion around risk versus benefit for alleviating osteoarthritis pain. This conversation is similar to discussing a team's standings or a player's stats to understand overall performance versus individual risk.

The core question for a veterinarian, according to Goldstein, is how best to alleviate a dog's pain. A holistic approach includes weight management and exercise. When medication is needed, vets must weigh the risks and benefits of all options, including nonsteroidal anti-inflammatory drugs (NSAIDs) and Librela. This decision-making process is as critical as managing a team's squad to ensure the right players are available for the right situations.

Goldstein noted that veterinarians have to have all the information they possibly can, emphasizing the importance of context. The same risk-benefit analysis must be applied to each individual pet. The identified cases are relatively advanced, leaving questions about early detection and management still unanswered. Zoetis plans to continue researching the condition to provide further guidance.
